FLU SHOT INFO NEEDED! If you are a KAISER PATIENT/ValleyCare INTERN, a consent form will go home tomorrow for your parent to sign and that will allow me to receive a fax with your flu shot info on it! (HIPAA compliance).
Everyone else, I need the info right away. Call your doctor's office tonight and leave a message if you have not taken care of this. The doctor will need your parent's consent. Make the call to make this happen. I am running out of time. I want my interns to ALL start on 11/26.
