My son/daughter ______________________ will be attending the CMAA Annual Meeting in Modesto, CA on Saturday, March 3, 2013. The conference will be held at the Sutter Gould Medical Foundation Education Center, 1700 McHenry Avenue.
I understand that there is no liability
insurance provided by Tri-Valley ROP regarding transportation to and from, and
at the conference for Medical Occupations ROP students. I will take full
responsibility for transportation/insurance, any and all liability regarding
transportation to and from this conference, and any and all liability while in attendance at the
